Healthcare: scribes, telemedicine and records
Use AI in your medical scribe without exposing the patient.
Rota transcribes the visit and removes the name, the CPF, the SUS card number, the CRM number and the record and health plan numbers from the text. The AI model writes the clinical note without knowing who the patient is, and your system links the note to the correct record.
Why this matters now
Resolution CFM No. 2,454/2026 of the Federal Council of Medicine has been in force since 26 August 2026. It allows AI as support for physicians and asks for governance, data protection under Brazil's General Data Protection Law (LGPD) and a record of AI use in the patient file.
Health data is sensitive personal data. When the full visit goes to an AI model, the patient's name, documents and history go with it, and the institution is accountable for that transfer.
The clinical content stays in the text: symptoms, tests, doses and decisions remain. Only what identifies the person is removed, and the AI still has what it needs to write the note.
How it works
- The audio arrives through the API Your application sends the visit recording, up to 25 MB, to the transcription route. The audio file is not stored.
- Rota transcribes and cleans The transcript comes back with placeholders instead of the name, CPF, SUS card, CRM, record number, health plan number and clinical dates.
- AI writes the clinical note Your system sends the clean text to a Rota model, through the OpenAI-compatible API, or to the model that you already use.
- The record receives the note Your system knows which patient the visit belongs to and saves the note in the correct record. The Rota dashboard keeps only the categories and counts of what was removed.
Example with fictitious data
Before Rota
Paciente Joana Ribeiro Matos, CPF 529.982.247-25, Cartão SUS 898 0012 3456 7890, prontuário 778201. Relata dor de cabeça há três dias, sem febre. Pressão 13 por 8. A Dra. Helena Prado, CRM-SP 123456, orienta dipirona 500 mg e retorno em sete dias.
After Rota
Paciente [PESSOA_1], CPF [CPF_1], [CNS_1], [PRONTUARIO_1]. Relata dor de cabeça há três dias, sem febre. Pressão 13 por 8. A [PESSOA_2], [CRM_1], orienta dipirona 500 mg e retorno em sete dias.
Fictitious text in Portuguese, processed by the API engine on 6 October 2026, with the placeholder policy.
How to integrate
Use the transcription route, in the OpenAI format, and then the chat route. If your system already talks to OpenAI, change the address and the key.
The chat route also goes through the barrier: if the text arrives with personal data, the barrier removes it before the model.

- Rota takes care of the data; the clinical decision stays with the physician.
- For long recordings, send the audio in parts of up to 25 MB.
FAQ
Frequently asked questions
Direct answers about this use. Did not find yours? Talk to the team.
Can AI write the clinical note without the patient's name?
Yes. The note depends on the symptoms, the tests and the decisions, which stay in the text. The name and the documents do not change the clinical content, and your system links the note to the correct patient.
Is the visit audio stored?
No. The audio file is not stored. When auditing is active, the input and the output stay encrypted for the defined period, with restricted access. The dashboard shows only categories and counts.
Does this meet Resolution CFM No. 2,454/2026?
Rota helps with the data part: it removes what identifies the patient before AI and keeps proof of the processing. The institution's governance, the record in the patient file and the physician's supervision complete compliance.
Which health data does Rota recognize?
Person names, CPF and CNPJ numbers, emails, phone numbers, addresses, SUS card, CRM, record, visit, authorization and report numbers, health plan and member card numbers, and clinical dates such as birth, visit and admission.
